Remember to fill in and post your consent form to us prior to your surgery. You will need to fast before your surgery. Our Day Surgery and staff are there to meet all your needs and assist you where possible.
One of our team will contact you to get your health fund and financial information, and any other information that may be required to ensure you have all your requirements met.

Before giving your consent to the procedure make sure you have received as much information as you require to make this decision.
If you have any queries or concerns regarding the nature of the procedure and anaesthetic, the reason it has been recommended, alternative actions and potential complications, it is highly recommended that you discuss this with your Surgeon or Anaesthetist prior to the day of procedure.
By consenting to the procedure you are also:
- Consenting to the taking of blood should an exposure injury occur to a staff member.
- Consenting to intervention should non-elective care be required in an emergency.
- Consenting to the use of medical information to enable optimal care be taken.
- Consenting to the provision of medical records to any specialist at this facility that is involved in your health care.
